- Health + Hospitals Senior VP — $25,000 fine Caused H+H to buy $98,000 in consulting services and ventilators from a company where her husband was a Senior Vice President, and used an emergency-issued COVID P-Card to spend $7,261 on food and alcohol at sit-down
- DOE Principal — $22,500 fine Directed $50,815 in school payments to her son's company for murals, business cards, banners, and workshops. The report cites invoice irregularities, including five workshops billed as held the same day and two with
- DOE Payroll Secretary — $10,000 fine Re-assigned her son in the DOE substitute system for six months of unauthorized work and entered payroll hours for 55 days he never worked. (Case 2023-514)
- Probation Officer — $7,250 fine Spent 200+ hours of city time on a private social-work consulting job, stored 87 work files on a city computer, and contacted city agencies for outside clients. (Case 2023-384)
- HPD Project Manager — $6,500 fine Stored 1,030 files for his private event-ticket business on his HPD computer and used his HPD email for it. (Case 2019-732)
